(4) samples of works (paper or electronic media can be submitted);

(5) description of the work (please write it from the three aspects of creation purpose, creation process and originality of

the work, and attach the signature and seal of the applicant, indicating the date of signature and seal);

(6) copies of power of attorney and identification documents of agents.
